To simplify the membership process for the Ontario Region, we have created an online Membership Form.

This form is in the pdf format and can be read with Acrobat Reader. If you do not have this program, click on the image below to go to the Adobe site for the free download.

To apply for membership; click here, to bring up the form. The form will open in its own window. Save it to your computer and print it out. The form and applicable fees can then be mailed to the address on the form.
